Dr. Josh Luke, FACHE
Hospital CEO, Author, USC Faculty

Dr. Josh Luke is a leading healthcare futurist designing graduate-level curriculum as Adjunct Faculty for the University of Southern California’s Sol Price School of Public Policy. Luke previously served as CEO of three acute hospitals, an acute rehab hospital, as Administrator for nursing homes and oversaw home health and hospice services as Vice President of Post Acute Services for a health system. Luke has a Ph.D. from the University of Nevada, and authored Readmission Prevention: Solutions Across the Provider Continuum and Ex-Acute: A former hospital CEO tells all on what’s wrong with American healthcare. Recent presentations include ACHE World Congress 2016  & 2017 and keynote presentations at the Case Management Society of America’s annual meeting and USC’s C-Suite Invitational.  Dr. Luke co-teaches a ACHE Cluster program on care coordination and has been a featured best-selling author by ACHE’s Health Administration Press for his book Readmission Prevention. He currently serves as CSO/Senior Health Policy Analyst for Nelson Hardiman Strategy.

Dr. Virginia Feldman

Dr. Virginia Feldman is a practicing surgeon and co-founder of multiple start-ups including an ambulatory surgery center. Dr. Feldman is the Co-Founder and CEO at Nexus Health Resources, which offers transitional care software and services. Its flagship software, NexusConnexions, allows clinical and non-clinical users to reduce the healthcare spend and improve patient behaviors outside of direct care. After receiving her MD degree from New York Medical College in 1991, Dr. Feldman completed residency training at St. Vincent’s Hospital and Medical Center and The New York Eye and Ear Infirmary. Since finishing her residency in 1997, she has been in clinical practice in Middletown, NY at Hudson Valley ENT, PC where she was Managing Partner from 2002 and President since 2006. She grew the practice to the premier Otolaryngology practice in the region. Strategically, Hudson Valley ENT, PC merged with ENT and Allergy Associates, LLP in October 2013.

Leah Throckmorton, MSN, RN-CCCTM
Director of PopHealth & 360 Services – Wise Health System
Care Coordinator and Clinical Health Coach

Leah Throckmorton is the Director of 360 Services for Wise Health System, located in Decatur Texas. The 360 Services department is a Transitional and Chronic Care Management team focusing on preventing hospital readmissions. She also services at the hospital’s ACO Care Coordinator and is a certified Clinical Health Coach. She was one of the original participants in Texas to obtain her board certification in Care Coordination and Transition Management. Leah received her Bachelors in Nursing in 2007 from the University of Texas at Austin and her Masters in Nursing Administration from Texas Tech in 2016. She has a passion for Population Health and enjoys creating new innovative programs that incorporate evidence-based practices. Leah resides in Alvord, Texas with her husband, Kris and her two children.
