SUMMARY:Quarterly Education Event - (3 Face to Face Credits Apply)
DESCRIPTION:PANEL 1: Integrating Acute Into Post Acute Care Settings\n(1.5 Face to Face Credits Apply to This Panel) \nThe Affordable Care Act has made no exceptions for the Acute Care Hospitals\, as they too are faced with ever more regulatory requirements and expanding financial penalties all as they strive to elevate the Standard of Care across the healthcare system. Though these requirements and penalties are not a new phenomena\, Acute Care Hospitals are in a new wave of financial penalties with Medicare payment reductions for readmission to acute care within 30 days of discharge. \nCome listen as our panelists discuss how Acute Care Hospitals are now\, more than ever\, working collaboratively with members of the post acute care continuum to cohesively manage patients. As we continue this path\, Long Term Acute Care Hospitals (LTACs)\, Inpatient Rehabilitation Facilities (IRFs)\, Home Health Agencies (HH’s)\, and Continuing Care Retirement Facilities (CCRCs) will play an even greater role in supporting our Acute Care Hospitals in the management of patient populations through the continuum of care. \nFEATURING : (CLICK HERE FOR FULL BIOS) \nModerator:\nMark Boles\, FACHE | Vice Chairman | Patient Advocacy Council\, Inc. \nPanelists:\nChet Bhasin | Service Line Director\, Rehabilitation | Texas Health Dallas\nJan Hamilton-Crawford\, FACHE | Vice President\, Operations | CHRISTUS Continuing Care\nStephanie Madrid | Market CEO | Kindred Dallas\nLoc Trieu\, MD | Regional Medical Director | Questcare Matrix \n\nPANEL 2: The Impact of the ACA:\nHow Readmission Penalties Will Affect the Healthcare Executive’s Mission\n(1.5 Face to Face Credits Apply to This Panel) \nThe cost of healthcare has been escalating over the past several years\, with expenditures surpassing $2.7 trillion in 2011. Trying to contain and reduce this growth has become a major policy focus not only at the federal and state levels\, but among employers and consumers as they struggle to keep up with the increasing cost of health insurance. Rising costs are placing an unsustainable strain on state and federal programs and individual and employer sponsored health plans. \nAccording to Centers for Medicare and Medicaid Services (CMS)\, hospital and physician services account for over 51% of the cost. So what are the drivers of cost in these areas?: Lack of coordination of services among health providers resulting in inefficiencies and duplication; unnecessary medical treatment\, clinical variation\, and costly medical errors; and a sky rocketing population of people with one or more chronic diseases. In effort to address some of these issues\, the Affordable Care Act (ACA) has reduced Medicare payments to hospitals with high 30-day readmission rates. \nJoin us as we discuss how readmission penalties are causing healthcare executives to get creative and consider new and innovative approaches to manage care transitions between community\, inpatient\, and specialty care settings while rethinking their financial strategy. \nFEATURING: (CLICK HERE FOR FULL BIOS) \nModerator:\nEric Jacobsen \, FACHE | Assistant Director | VA North Texas Health Care System \nPanelists:\nReggie Allen\, MBA\, RN | System Director\, Quality and Clinical Operations | CHRISTUS Health\nBobby Barajas| Vice President Sales & Marketing| Loopback Analytics\nBrenda Blaine\, DNP\, RN-BC\, FACHE\, NEA-BC | CNO/COO | Baylor Medical Center at Irving\nCynthia Hanna\, RN | CEO | Tarantino Healthcare Consultants \n3.00 – 3.30 Registration\n3.30 – 5.00 Panel 1\n5.00 – 6.00 Dinner and Networking\n6.00 – 7.30 Panel 2 \nCatering provided by: \n \n \nREGISTER TODAY!

SUMMARY:Listening to Employers
DESCRIPTION:Listening to Employers: How Health Systems Can Support Population Health Management  \n1.5 face to face credits apply \nThe continued pressure to manage healthcare costs and maximize workforce productivity has caused employers to broaden their participation in population health management for their employees. Managing the health of a defined population requires attention to issues of access\, cost and quality. In addition to traditional work injury prevention and management programs\, employers are expanding the services offered to employees to include health promotion\, health behavior change\, primary care\, and other allied health services. Employers may contract with health plans\, with niche healthcare providers or with local health systems to design and offer health services that meet the needs of employees while achieving the goals of the employer. This program focuses on understanding the needs of employers in managing workforce health and discusses how healthcare organizations can partner with and support employers in population health management. \nFeaturing:\nCarla Dawson | VP\, Total Health Total Rewards | Texas Health Resources \nLeia Spoor\, MPH | Director\, Health and Wellness | Baylor Scott & White Health \nMatt Morris\, MBA\, CBC | President | Gus Bates Insurance & Investments \nModerated By:\nKirk King\, FACHE | President | Texas Health Arlington Memorial Hospital \nEVENT INFO & REGISTRATION HERE
