SUMMARY:2015 Interprofessional Practice Symposium: Putting Theory into Practice
DESCRIPTION:The goal of this symposium is to promote collaboration between academics\, the practice community and supporting health care agencies and organizations to highlight practical aspects of collaborative practice with a focus on academic and practice models demonstrating evidence of improved patient experience\, outcomes and safety. The symposium includes a range of educators and practitioners discussing leading practices and case studies in both a large group and workshop style settings. \nInnovations to improve quality of care and patient safety are a topic of discussion and research within all health professions. Effective interprofessional health care teams have been identified as a factor in improved quality of care and patient safety. Training health care professionals\, health professions students\, patients and their families to function effectively as members of a collaborative team is a common goal across the practice and academic communities and supporting healthcare agencies and organizations. It is vital that the practice and academic communities and supporting health care agencies and organizations work collaboratively on development and dissemination of innovative models supporting collaborative practice. \nBest practices supporting effective collaborative team delivery of care must translate from theory to practice highlighting viable models for dissemination. Increasing exposure between the practice and academic communities provides health professions students with clinical setting examples of effective collaboration. This also informs practitioners of the team training models students are being exposed to and how to carry them into their future practice. \nMultiple continuing education credit types have been awarded for this activity.  Register today!
